General Information
Thank you for coming to speak/perform at Mizzou!
A student organization has obtained some funding for your event, so we need to collect some information for the University's Accounting and Legal departments. This information will be used to prepare your payment, and, if you are receiving over $500, to prepare a contract.
If applicable, the contract will be emailed to you in advance of your event--you must sign and return it in a timely manner to ensure that your payment is not delayed.
Please note that you will receive one lump-sum payment shortly after your event; we do not coordinate travel plans or reimburse individual expenses. The information you submit is encrypted and will be directed to an MU staff member.
Any forms should have a real signature not a typed signature for processing purposes.
